Does your state or country require you to keep assessments or evaluations you’ve put together for each child? The three states I’ve homeschooled in have had this requirement in one form or another.
The individual states, or the homeschool accountability association/umbrella school, have not provided a specific outline or format that they required these to be in. The policies have just stated that some record keeping and annual review of some kind needed to be kept “on file” at home. I’ve never had to send anything in.
For the first 6 years I used my own format and it worked really well. Most years I completed an initial 90-day assessment, and then completed another one after the second 90-day period to wrap up the school year. Last year I just did one for each child for the whole year all at once. At this point, I personally compose mine in a narrative format and there is no grading component added.
This is essentially the same information I include in my own, and I’ve made it easily editable for use in your homes!
